Visits are by appointment only. Patients who do not provide a 25 hour cancellation notice will be charged in full for their scheduled appointment.

Medication Management	5-15 minutes	$65
Intermediate Follow Up	20-30 minutes	$130
Extensive Follow Up	45-60 minutes	$200
Family Appointment w/o patient	5-15 minutes	$65
Family Appointment w/o patient	20-30 minutes	$130
Extensive Phone Call	20-30 minutes	$50
